The purpose of this questionnaire is to facilitate the self-evaluation of the employee. The questionnaire is based on the different competences required within the function performed. The same questionnaire is sent to you as the (functional)manager of the employee, however answering the "open" questions is not mandatory. Giving a score to all specific competences (last page of the questionnaire) is mandatory before submission.
